Amerigroup Community Care of Georgia recently visited Beacon Pediatrics, Henderson Family Practice, Dr. Lina Heath Harper, and Southeast Pediatrics in Waycross as part of its statewide Rural Health Program, developed in partnership with GoMo Health.
The visit brought together Amerigroup representatives, local healthcare providers, and community stakeholders to discuss the unique healthcare challenges facing residents of Ware County.
Among these were access to preventive care, chronic disease management, maternal health services, transportation barriers, and continuity of care.
“Building relationships with local providers is an important part of our work,” said Mel Lindsey, president of Amerigroup Community Care. “Each community is different, and by partnering with local providers, we can identify and address the healthcare challenges these communities face more effectively.”
The rural health program is an initiative focused on improving healthcare access and outcomes for Medicaid members living in underserved rural communities across Georgia.
During the visit, Amerigroup highlighted resources and tools designed to strengthen care coordination, support healthcare providers, and connect individuals with services that can help improve their overall health and well-being.
Attendees also received demonstrations of provider support resources and personalized digital engagement tools that help patients stay connected to care between appointments, improve treatment adherence, and close gaps in care.
“Continuing to have conversations with local providers is critical,” Lindsey said. “Their local perspective helps us pinpoint solutions that positively impact the lives of rural Georgia patients.”
The visit to Waycross was one stop on a tour that aims to engage more than 100 healthcare locations and 1,000 providers across the state in support of more than 450,000 Medicaid members.
At the center of the initiative is a commitment to extending care beyond traditional clinical settings, helping providers better manage population health while supporting members as they navigate the healthcare system and stay on track with their care plans.
